Appendix A

These forms can also be found in Appendix A of a physical copy of the NCWSC Guidelines Binder.

This page contains forms used by the NCWSC in the conduct of its business. This is the list of forms currently used along with the title of the member responsible for maintaining the form. Click on the corresponding PDF or DOC links to view and download the form.

You can now fill out most, eventually all, PDF forms electronically using Acrobat Reader as follows:

1. Open the form on your computer by clicking the “PDF” link.
2. It should open automatically with Acrobat Reader (the latest version is helpful), either in the browser window or in Acrobat Reader directly.
3. Type your information into the fields desired. If there are no fields, the document has not been converted to an electronic version yet.
4. Move from one field to the next by using the “tab” key on your keyboard, or by clicking on the next field of interest.
5. Any “Totals” on the form should be calculated automatically. If not, the form did not get opened with Acrobat Reader. If you need technical help, Contact Us.
6. When the form is filled out to your satisfaction, save the file to your computer.
7. You can print a copy of the form using your normal procedure for printing pages. You could also “snail mail” a printed copy.
8. You can now attach the file to an email addressed to the correct Officer or Coordinator / Committee Chair.

Those forms without links are not available on line. To receive a copy, please contact the Officer who is responsible for maintaining the particular form.

Sort the entire table by clicking on a column’s title. Search the table by typing in keywords in the search text box. Results will be shown as you type.

Get Acrobat Reader. These guidelines, forms and bylaws are in a .PDF format, which requires the use of a free software program called Acrobat Reader to download them. A connection to the site that offers the Acrobat Reader is with the understanding that including this connection does not imply that NCWSA, any other service arm in Northern California or Al-Anon Family Groups, Inc. endorses this or any other product supplied by the manufacturer of the Acrobat Reader.
